Signify Health, part of CVS Health , is hiring full-time clinicians to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial health plan members. In this role, you will:

• Conduct pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area (majority of visits scheduled weekdays between 8 a.m. and 7 p.m.)
• Provide 40 hours of availability weekly, consistently providing 160+ capacity units per month
• Execute a strong visit completion rate, consistently pacing 110+ completed visits per month Other role expectations and requirements:
  • Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
  • Consistently engage with regular manager, team and clinical meetings
  • Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics
  • Conduct Video Visits as needed (maintain training, affiliation, and compliance with Video Visit requirements, regular training required)
  • Maintain training and complete all diagnostic and preventive service (DPS) products as directed (regular training required)
  • Maintain all required supplies and associated diagnostic equipment related to DPS offerings
  • Maintain active state licenses in coverage areas
  • Maintain board certification (as applicable)
  • Maintain multi-state licenses (preferred)
  • Experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format (preferred)
  • Be flexible to travel locally and within licensed states. May require hotel stay, car rental, and/or flight up to 20% annually
  • Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies (including, but not limited to, iPad, medical supplies and scale), required
  • Accommodate field observations and/or "ridealong" experiences, as requested During our In-Home Health Evaluations (and other associated follow-up visits), you'll meet people where they are and re-engage them with their health care during an approximately 45-minute pre-scheduled visit. The visit includes a medication and medical history review, a physical evaluation, and, if ordered by the person's health plan, additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.). You will be provided a pre-populated, patient-specific form for each evaluation and are not required to prescribe medicine, order lab tests, or alter people's current treatment regimen.
